Abstract
Purpose: The aim of this study is to focus on the use of ring fixation technique for the management of infected lower limb fractures. War injuries are one of the causes of extensive bone injuries. This is particularly common in lower limbs and more so in countries with random availability of weapons. Methods: Between 2015 until 2019, 7 patients were brought to our facility who had sustained war injuries particularly lower limbs, some with bone defects and associated soft tissue loss. All cases were managed by early aggressive debridement of all devitalised tissues back to the bone and treated by Ilizarov ring fixator at some point of time. Results: The overall bone results were satisfactory in 6 out 7 patients with remaining 1 still under treatment. Functional results were good in all 7 patients who were able to ambulate independently with assisted support device. The functional end results were significantly affected by infection and other associated injuries as well as damage to the joint articular cartilage at the time of injury. Limb length discrepancy <3cm was present only in one patient which was managed by a shoe raise. Conclusion: Ring fixation is a safe, versatile, adaptive and effective method in treatment of complex infected lower limb fracture. It allows early weight bearing thus improving overall patient satisfaction.
